PHS Health Screenings:

All 10th Grade students and transfer students will receive a vision and hearing screening as is required by Virginia state law. Parents will be notified of any deficiencies in these areas and follow-up with a healthcare professional may be recommended.

PHS Medication Administration:  Students may receive prescription medications at PHS if the following procedure is followed:

• A medication administration consent form is completed by the parent

• A written order from the physician is brought or faxed to the clinic prior to the medication being given

• The medication is in the original, dated container

Students may receive over the counter medications at PHS if the following procedure is followed:

• A medication administration consent form is completed by the parent

• The medication is in the original packaging

You may obtain a copy of the Medication Administration Consent Form and a copy of the Division Medication Policy from the school clinic.
